Date of Follow-up Visit: 19 July 2026
Location: Kobilmahagu, Tamale, Northern Region, Ghana

Introduction

As part of its commitment to improving the health and nutritional status of vulnerable children, the KOKOLANA Foundation conducted a follow-up visit to Baby Nashira on 19 July 2026. The visit aimed to assess the child’s progress following the Foundation’s donation of food items and medications, evaluate the effectiveness of the nutritional intervention, and provide continued support and guidance to the caregiver.

Team Members

The follow-up visit was conducted by:

* Mr. Muksin
* Mr. Fataw
* Miss Maridia
* Miss Zainab
* Mr. Malik
* Miss Ayisha
* Miss Latifa
* Mr.Zakaria
* Miss Hamdia

Findings

The team observed remarkable improvement in Baby Nashira’s overall health and nutritional status. Compared to the previous visit, the child had gained noticeable weight, indicating a positive response to the nutritional intervention.

Baby Nashira has also developed improved standing skills and is making encouraging attempts to walk, demonstrating significant progress in her physical growth and motor development.

Furthermore, the severe wasting previously observed had reduced considerably. The hollow portions around the clavicle, which had been prominent during the initial assessment, were now adequately filled, reflecting improved nutritional recovery. The child was also noticeably more alert, active, and responsive, while significant hair growth was observed, further indicating an improvement in her overall nutritional status.

The team was pleased to observe that the caregiver had maintained a clean and hygienic environment as recommended during the previous visit. Good environmental hygiene was evident throughout the household and continues to play an important role in Baby Nashira’s recovery by reducing the risk of infections and promoting healthy growth.

Assessment of Remaining Support

The team engaged the caregiver to determine whether the donated food items and medications were still available and to estimate how long they would continue to meet Baby Nashira’s nutritional needs.

The caregiver confirmed that the remaining food supplies and medications were still available and estimated that the food items would adequately cater for Baby Nashira’s nutritional needs for approximately the next three weeks.

Caregiver’s Feedback

The caregiver expressed profound appreciation to the KOKOLANA Foundation for the timely intervention and continuous support. They stated that the Foundation’s assistance had played a significant role in restoring Baby Nashira’s health to a normal or near-normal condition.

In expressing their gratitude, the caregiver offered the following prayer:

“May Allah richly bless KOKOLANA Foundation.”

Conclusion

The follow-up visit demonstrated the positive impact of the KOKOLANA Foundation’s nutritional intervention on Baby Nashira’s recovery. The significant improvements in weight gain, physical development, alertness, hair growth, and reduction in wasting are clear indicators that the intervention has been successful. Equally commendable is the caregiver’s commitment to implementing the nutritional and hygiene recommendations provided during the previous visit.

Although Baby Nashira has made remarkable progress, the Foundation recommends continued nutritional support, regular growth monitoring, and periodic follow-up visits until she fully attains a healthy nutritional status. KOKOLANA Foundation remains committed to supporting vulnerable children and their families through sustainable health and nutrition interventions that improve the quality of life within our communities.
